Rush Legal utilizes templates for certain types of Research Agreements to ensure standardized language and aid in efficient execution. These templates are available for you to download and share with sponsors, collaborators, and team members.

Data Use Agreement (DUA) for limited data set research
Data Use Agreement (DUA) for recipients of de-identified data set(s) originating from Rush
Data Use Agreement for recipients of data containing protected health information (PHI) that originated from Rush
Data Transfer Agreement (DTA) for basic science data used when Rush researchers are transferring basic science data (i.e. no human subject data) to another institution
Material Transfer Agreement (MTA) where Rush is transferring material to another institution
Uniform Biological Material Transfer Agreement (UBMTA) used when Rush is transferring biological material to another non-profit institution and that may be subject to patent protection
Uniform Biological Material Transfer Agreement Implementing Letter to provide record of the Biological Material Transfer
One-way Non-Disclosure Agreement (NDA) where Rush is disclosing otherwise confidential, sensitive, or valuable information to a recipient
Mutual Non-Disclosure Agreement (NDA) where Rush and a business partner are both sharing confidential, sensitive, or valuable information with each other.
